Who we are looking for: Detail-oriented Certified Nursing Assistants (CNA), Medical Assistants (MA), Licensed Practical Nurses (LPN), and Registered Nurses (RN) to support Care Managers and UWH providers by managing dashboards, tracking referrals, and keeping communication moving across post-acute settings. This role includes patient and family outreach and coordination of services to support smooth transitions of care. This position does not involve independent clinical assessments. Main Responsibilities (may include but are not limited to): Maintain care coordination dashboards, patient lists, and basic tracking for CCM/PCM programs Track referrals, authorizations, and follow-up appointments across specialty and post-acute services Conduct non-clinical patient and family outreach, including welcome calls and appointment reminders Coordinate communication between patients, facilities, Care Managers, and providers Gather, upload, and organize medical records, wound photos, labs, and imaging reports Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments Complete clerical tasks related to Transitional Care Management (TCM), including outreach, scheduling, and documentation Monitor admissions, discharges, and census changes from skilled nursing and post-acute facilities Prepare patient summaries, reports, and materials for care team meetings Escalate clinical concerns promptly to a Care Manager or provider Participate in required training, meetings, and compliance activities Provide excellent customer service and follow communication and organizational standards Perform other duties as assigned Skills required to succeed: Active license/certification in Washington State Computer savvy; Intermediate proficiency using MS Suite: Word, Teams, Outlook, and Excel Professional phone etiquette with excellent customer service skills Excellent attention to detail, strong organizational skills and ability to manage time effectively Must have strong interpersonal skills and the ability to communicate effectively with a variety of people through both verbal and written communication lines Ability to prioritize workload and have a strong sense of urgency when time-sensitive situations arise Ability to multitask while providing compassionate patient care Leadership qualities with the ability to work well within a team of healthcare professionals Must be self-motivated, punctual, and dependable Must be trustworthy, honest, and have a positive and professional attitude Compensation: $21.00 - $27.50 hourly – DOE Location: Puyallup, WA This position is classified as: Hourly, Non-Exempt ; Full-Time employment Hours: Typical hours are Monday through Friday, 8:00am to 4:30pm, though hours may vary; no nights or weekends Benefits: Associates working 20 hours per week: Medical/Dental/Orthodontic/Vision/RX – minimum 80% towards employee monthly premiums covered, HSA matching, dependent coverage available at employee’s expense Employer Sponsored Life, AD&D, and Disability Insurance Voluntary Supplemental Insurance: Accident, Cancer, Critical Illness, STD, Life, Identity Protection, Financial Wellness, and Legal Plan coverages available Paid Time Off: PTO accrual up to 132 hours (16.5 days) your first year of employment, based on 1.0FTE status 8 paid Holidays for full-time employees 401(k) match on first 4% Core Values that promote work-life harmony No weekend or call Work with amazing people who have created a culture where we recognize each other’s wins and don’t tolerate gossip or drama *Do you want to grow personally and professionally by working with the best?
